VIII. Notice regarding Alignment Health ACO, LLC
The participation agreement between CMS and Alignment Health ACO, LLC, will not be renewed for 2026. Therefore, effective as of midnight on 12/31/2025, providers currently in Alignment Health ACO, LLC, will no longer participate in Alignment Health ACO, LLC, a Medicare Direct Contracting Entity.
Your Medicare benefits have not changed. Your doctors will not change due to this notice, and this notice does not impact their ability to contract with Medicare. You still have the freedom of choice to go to any doctor, hospital, or other healthcare provider of your choice that accepts Medicare.
As a beneficiary of a participating physician in the ACO REACH program you may have had access to additional Benefit Enhancements or Beneficiary Engagement Incentives, implemented by Alignment Health ACO, LLC, and will be terminated or otherwise cease to be in effect as of December 31st, 2025. Furthermore, any services furnished under the Benefit Enhancement will no longer be covered by Medicare as of March 31st, 2026, and the Beneficiary may be responsible for the payment of such services.
